A geriatric care manager is a health and human services specialist. As geriatric care specialists we assist families who are caring for older relatives or friends. The goal of the care specialist is to assist older clients attain maximum functional potential, based on the wants and needs of the client and the family, overall stress is reduced and positive decision making is increased.
When possible, remaining at home is the best possible option, and is one of the first things that we explore. In the home we will provide medication management, nurse aides, meal preparation, housekeeping and laundry, transportation, and companionship. Assessing your loved one for Medicare covered options such as: home health therapy, and hospice services are key to providing maximum function in the home.
If remaining in the home is not an option, we will provide guidance in choosing a quality nursing or assisted living facility. Location, level of care, and finances are all part of the decision making process when choosing the appropriate living arrangement for your loved one. If there is a situation that requires the need for a guardian, conservator, or power of attorney, we will direct you to specialists in the community who will work through the legalities with you; with us by your side.
